Job Title: Licensed Practical Nurse, Home Health

Job Location: Lima, OH

Job Type: Contract - Traveler - 13 weeks

Job Duties:

• Provide skilled nursing care to patients in their homes under the supervision of a registered nurse or physician.
• Perform comprehensive assessments of patients' health status and develop individualized care plans in collaboration with the healthcare team.
• Administer medications, treatments, and procedures as prescribed by the physician and according to established protocols.
• Monitor patients' vital signs and symptoms, assess their response to treatment, and make appropriate interventions as needed.
• Educate patients and their families on disease management, medication administration, and self-care techniques to promote independence and improve outcomes.
• Coordinate with other healthcare professionals, including physicians, therapists, and social workers, to ensure continuity of care and facilitate interdisciplinary collaboration.
• Document patient assessments, interventions, and outcomes accurately and timely in the electronic medical record.

Job Requirements:

• 3 Years of Home Health Experience
• Home Care HomeBase EMR Experience - Required
• OH State License
• BLS (AHA)
• Covid Card

Schedule:

• Day Shift
• 5x8
• 40 hours guaranteed in a week
