Are you someone who enjoys working with patients and families to identify, examine, and treat mental and emotional issues and disorders? Do you have a master’s in counseling, psychology, social work, or a related mental health field from an accredited institution? If so, then we would like to speak to you! 

We are currently seeking a CoCM Case Manager/Licensed Therapist to join our 26Health team. In this roll you will have the unique opportunity to make contributions to both the Collaborative Care Management and Behavioral Health Teams. In both the licensed therapist and CoCM Behavioral Health Case Manager roles, you will be an integral part of our non-profit organization.The CoCM BH Case Manager is responsible for supporting and coordinating the mental and physical health care of patients identified as those who could benefit from the services of the CoCM Program including providing brief behavioral interventions as well as ensuring effective communication between the patient’s team of providers. The CoCM BH Case Manager also plays an important role in 26Health’s multi-year grant with SAMHSA by gathering data regarding participants’ knowledge, behaviors, and attitudes about substance use and sexual activity and identifying those who may be at risk to provide care and education. In your role as a licensed therapist, your primary responsibility will be to provide evidence-based treatment to individuals, couples, families, and groups.

Duties and responsibilities

  • Completes biopsychosocial assessments and submits clinical documentation within specified timeframes.
  • Utilizes a registry to track patient follow-up and clinical outcomes, identifying and re-engaging patients as necessary.
  • Systematically tracks treatment response and monitors patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications. 
  • Utilizes evidence-based modalities to meet treatment goals and provides self-management tools for symptom improvement.
  • Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate.
  • Collaborates with a multidisciplinary team for comprehensive patient care.
  • Facilitates support groups and workshops for the LGBTQ community and other identified groups/communities
  • Supports psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment.
  • Coordinates patient care with medical providers and other health professionals, facilitating treatment plan adjustments when needed.
  • Participates in regularly scheduled (usually weekly) caseload consultation with the psychiatric consultant and communicates resulting treatment recommendations to the patient’s medical provider.
  • Facilitates treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultant and who may need more intensive or more specialized mental health care.
  • Develops and completes relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the caseload.
  • Encourages patients to express feelings, gain insight, and develop healthier relationships.
  • Demonstrate cultural competence, trauma-informed care, and experience with LGBTQ populations.
  • Reviews completed SAMHSA Questionnaires to identify participants who would benefit.

  • Facilitates SAMHSA Exit and Follow-up Questionnaires with participants.
  • Facilitates referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse PHP, IOP, inpatient treatment)
  • Complies with legal responsibilities for reporting child and elder abuse.

Qualifications/Knowledge/Skills/Experience

  • Master's or Doctoral degree in Counseling, Psychology, Social Work, or a related mental health field from an accredited institution 
  • Valid and active state license to practice psychotherapy in Florida (LMHC, LCSW, or LMFT)
  • Ability to effectively work in a complex team environment and to collaborate with both external and internal professionals and other customers.
  • Excellent verbal and written communication skills; communication style that is open and fosters trust, credibility, and understanding.
  • Ability to maintain effective and professional relationships with patients and other members of the treatment/care team.
  • Trained in at least one of the following techniques is preferred: EMDR, DBT, Play
  • Therapy, Trauma Focused-CBT, Motivational Interviewing, IFS, or Brain Spotting
  • Demonstrated experience providing therapy and/or counseling services with 

strong awareness in matters of cultural competence

  • Bilingual proficiency in Spanish, strongly preferred.
  • Ability to effectively engage patients in a therapeutic relationship.
  • Ability to work with patients by telephone and/or telehealth, as well as in person.
  • Experience with assessment and treatment planning for common mental health and/or substance misuse disorders
  • Basic knowledge of psychopharmacology for common mental health disorders that is within the appropriate scope of practice for the type of provider-filling role

Working conditions

  • Prolonged periods standing, walking, and/or sitting at a desk and working on a computer may be required. Work may be required in various environments as needed by project/activity.

  • May require the occasional ability to lift to 25 pounds.
