Department: 34100 Pediatric - Nursing Status: Full time Benefits Eligible: Yes Hours Per Week: 40 Schedule Details/Additional Information: 1st shift, M-F, rotating weekend and holidays Major Responsibilities: Assumes primary responsibility for a patient caseload. Coordinates all services and collaborates with members of the interdisciplinary team to provide consistent patient care. Conducts initial assessment and establishes plan for treatment to obtain thorough physical, psychosocial and environment data; establishes and follows a care plan which identifies problems, (nursing diagnosis) and evaluates outcomes within a certain time frame. Communicates with physicians and other team members to confirm, correlate and update patient plan of treatment and provides physician with a status report on a regular basis. Provides on-going assessment of patient response to treatments, medications and teaching and applies interventions as appropriate. Updates and revises care plan as indicated. May delegate this responsibility to staff nurse. Utilizes Care Management Initiative material consistently where appropriate and available. Schedules case conferences with manager and other members of the interdisciplinary care team on an as needed basis to discuss case management issues. Instructs and supervises paraprofessional staff on a timely basis and in accordance with agency, state and federal regulations. Performs Home Health Aide supervisory visits. Facilitates the discharge planning process according to agency standards and utilizes interagency/system and community resources to assure continuity of care after discharge. Maintains and updates accurate clinical and patient records according to agency, state and federal guidelines. Participates in Continuous Quality Improvement activities and assures that quality and compliance standards are not compromised. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards. Licensure, Registration, and/or Certification Required: Registered Nurse license issued by the state in which the team member practices, and Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required, and A valid drivers license issued by the Division of Motor Vehicles, or Driver license issued by the state of Illinois. Education Required: Associate's Degree in Nursing. Experience Required: Typically requires 3 years of experience in clinical nursing that includes experiences in home health or hospice. Knowledge, Skills & Abilities Required: Excellent awareness and perception skills, for the purpose of assessing a patient's needs, concerns and to recognize changes in their health condition. Proficiency in clinical skills with the ability to work under direction and make sound judgments. Excellent communication and interpersonal skills. Ability to work with a variety of clients with patience and respect. Must have a tolerance for differences and an appreciation of multi-culturalism and diversity of the patients and their families. Excellent decision making and critical thinking skills, with the ability to work independently and make sound judgments. Excellent prioritization, organizational and problem solving skills. Must possess and provide proof of a valid Driver License and a registered, functional car with adequate insurance coverage. Physical Requirements and Working Conditions: Must be capable of the manual dexterity required to use and operate a variety of medical equipment and supplies, writing implements and office equipment. Must have the physical agility to move about in confined spaces, including bending, twisting, kneeling and squatting. Must be able to work safely with people of various size and weight and diverse and complex medical conditions and physical limitations. Must have the physical stamina to attend to patient care requirements over a period of three or more hours without complete rest. Must be able to concentrate with frequent interruptions and remember multiple tasks and functions over a period that extends from minutes to hours and frequently weeks at a time. Must have accurate vision to read gauges and measuring devices and to interpret written instructions. Must be able to: - Lift up to 50 lbs. from floor to waist. - Lift -up to 20 lbs. over the head. - Carry up to 40 lbs. a reasonable distance. Must be able to: - Push/pull with 30 lbs. of force. Perform a sliding transfer of 150 lbs. with a second person present. Operates all equipment necessary to perform the job.
