Thank you for considering a career at Mercy Health Job Summary: Provides evaluation, reassessment, and treatment of patients referred for home health physical therapy and supervises the plan of care and clinical work of the PTA's assisting with the care. Serves as the home health case manager on therapy only cases taking responsibility for leading the care team, managing the total care plan and assuring adherence to the regulatory and agency guidelines for these cases. Job Functions: Completes accurate evaluations, reassessments and treatments of physical therapy patients. Oversees PTA treatments assuring adherence to plan of care and quality outcomes. Develops patient plan of care and adapts the plan according to assessment data and the patient needs. Schedules work independently and according to patient's clinical need and agency policy. Documents care, progress, goals, contacts, and orders according to policy and regulations. Serves as care team member, communicating effectively with patients, families, caregivers, physicians, and third party payors. Works autonomously without direct supervision in the field. Uses computerized technology for clinical care effectively after on the job training. Serves as primary case manager on therapy only cases.
