Hours: Must be available Monday Friday 8am-5pm (some on call as needed)

Key Responsibilities And DutiesIn this role, you'll be at the forefront of guiding our clients towards their best state of wellness, leading a team of caregivers with precision and compassion. Here's what you'll be responsible for:
  • Holistic Assessment: Oversee the comprehensive assessment and evaluation of clients' needs, working closely with families to achieve and maintain optimal wellness.
  • Resource Facilitation: Ensure clients receive the right resources and support for their care and wellbeing by facilitating interactions with various providers.
  • Strategic Decision-Making: Take charge of decisions related to resource allocation, approach, and tactical operations within your department, driving forward key projects and initiatives.
  • Collaboration Champion: Engage in cross-departmental collaboration, participating in meetings with fellow team members to align our efforts with company objectives.
  • Regional Connectivity: Maintain regular contact with other care managers across the region, fostering a network of knowledge and support.
  • Team Stewardship: Lead the recruitment, onboarding, and, when necessary, the termination of caregivers within your scope, ensuring our team is built on excellence.

QualificationsTo excel in this role, you should bring the following qualifications:
