- M-F 8am-4pm
- The pay for this position is $60.00-$70.00 an hour depending on experience
- Benefits are available to full-time employees after 90 days of employment
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
- Coordinates patients through the acute care hospitalization
- Manages all aspects of discharge planning/transition process
- Performs utilization management and quality screenings
- In- Patient Case Management is required
- Licensed RN in NJ
- BSN required
- 2 years of acute care or inpatient care management or managed care
