Case Manager, Elders' Storytelling Program

The Case Manager uses culturally sensitive, evidence-based techniques in life review and storytelling to help elderly clients reduce depressive symptoms and regain social connectedness with their family, friends, caregivers, and community. Additionally, the Case Manager collaborates with Behavioral Health Counselors to enhance the quality of services and outcomes for isolated older adults at risk of mental health issues or with unrecognized symptoms.

Duties and responsibilities

  • Provide culturally sensitive, client-centered individual, group rehabilitation services and family collateral services.
  • Identify outreach opportunities and conduct outreach in a community setting.
  • Screen clients for program eligibility and assess their needs.
  • Attend community sites or home visits to facilitate clients' storytelling activities.
  • Facilitate referrals to community resources and provide case management services.
  • Assist clients in establishing and nurturing social and community support networks tailored to their needs.
  • Identify methods to document the older adult clients’ oral stories as keepsakes for sharing with family, friends, and community.
  • Conduct pre-and post-intervention data collection using standardized assessment tools for depressive symptoms, loneliness, and life satisfaction.
  • Complete timely and accurate documentation in adherence to Medi-Cal, Santa Clara County, and AACI standards.
  • Provide translation and cultural brokerage.
  • Participate in clinical supervision, consultations, and training.
  • Accompany clients to appointments in the community as needed.
  • Complete other duties and related projects as assigned.

Qualifications


Qualifications

Education & Experience:

  • BA/BS degree in psychology or a related field, or equivalent training and experience that would provide the ability to perform the duties described above.
  • One year or more of case management related experience.

Knowledge, Skills, & Abilities:

  • Knowledge of community and county resources to facilitate referrals and coordination with appropriate agencies.
  • Proficient in using commonly used technology tools and devices including computers, email, and internet.
  • Working knowledge of Microsoft Office.
  • Ability to maintain a valid California driver license, current personal auto insurance, and an MVR sufficient to maintain insurability under agency auto liability. 
  • Verbal and written fluency in English required; Bilingual skills in a second language (particularly Spanish, Mandarin, Vietnamese).

Competencies:

  • Ability to communicate well with people of diverse cultural professional and experiential background.
  • Ability to establish and maintain effective work relationships as part of a multi-disciplinary team.

Working conditions

This job requires you to be in the community, in client’s homes, and other community partner sites. Work is often conducted in an office environment. Basic safety precautions and use of protective clothing or gear may be required. Working with clients who are seriously mentally ill, or high risk may increase exposure to secondary trauma. Driving is essential to the function of this job. The job requires travel up to 25% of the time.
