Compensation: $65 per visit, plus stipends, plus on call pay.
About Us: At Abundant Hospice, we provide compassionate end-of-life care that respects the dignity and wishes of our patients. We are seeking a dedicated social worker to join our team and support our patients and their families through their hospice journey.
Job Description: The Part-Time Social Worker will provide psychosocial support to patients and families, assist with care planning, and connect them with community resources.
Responsibilities:
- Conduct initial and ongoing psychosocial assessments of patients and families.
- Develop and implement individualized care plans.
- Provide counseling and emotional support to patients and families.
- Assist with advance care planning and end-of-life decisions.
- Coordinate with interdisciplinary team members to ensure comprehensive care.
- Facilitate access to community resources and support services.
- Participate in case conferences and team meetings.
Qualifications:
- Master's degree in Social Work (MSW) from an accredited institution.
- Licensed Clinical Social Worker (LCSW) preferred.
- Minimum of 2 years of hospice or palliative care experience.
- Strong understanding of hospice philosophy and regulations.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
Compensation:
- $65 per visit.

Pay: $61,784.11 - $74,406.66 per year
Expected hours: 10 – 30 per week
