**Overview:**

We are seeking a dedicated Care Transition Coordinator to join our team. The ideal candidate will have a strong background in home health/hospice and medical settings, with a focus on patient care coordination and support.

**Duties:**

- Coordinate patient referrals to ensure appropriate and timely services

- Utilize physiology knowledge to assist in patient assessments

- Maintain accurate medical records and update patient information

-Sales, marketing, clinical liasion

- Collaborate with healthcare providers across various specialties including hospital, home helath, home care, hospice and community organizations.

**Experience:**

- Previous experience in healthcare coordination/martketing roles preferred

- Knowledge of medical terminology and procedures

- Familiarity with electronic health records (EHR) systems

- Strong interpersonal skills for effective communication with patients and healthcare professionals

- Ability to multitask in a fast-paced environment

- Understanding of insurance verification processes

-Minimum of 1 year RN experience required

Pay: $25.00 - $50.00 per hour

Expected hours: No less than 40 per week
