Access-Supports for Living is Hiring a Program Associate Near Newburgh, NY

Hours: Mon-Fri - 8:30 am - 5pm
Location: Newburgh, NY
Rate of Pay: $18-21
1x weekly to Middletown/Newburgh and 1x per month to Mount Vernon
OVERVIEW OF PRIMARY RESPONSIBILITES:
The program assistant is generally the ambassador of first impressions with the ability to demonstrate a pleasant and professional demeanor to meet, greet, and manage all in-person and telephone contact. As the initial point of contact for daily inquiries, requests and follow-ups by members, families and co-workers, the Program Associate must be able to adapt to changing tasks and priorities.
PRIMARY FUNCTIONS:
  • Coordinate program documents (i.e. closing and archiving charts) to ensure compliance with regulatory requirements and protocols
  • Communicate and respond to all calls and other correspondence regarding the care management program and services (i.e. referrals, scheduling intakes, general information)
  • Ensure proper management of file maintenance in EHRs (Foothold CM, Net Smart) and record retention protocols
  • Coordinate and communicate directly with Health Homes and other referral sources
  • Provide support to care management teams at all locations (Middletown, Newburgh, Mount Vernon)
  • Heavy utilization of office and technology equipment, to include computers, scanners, fax machines and phone routing
  • Travel to other care management program locations to provide support
  • weekly to Newburgh / Middletown
  • monthly to Mount Vernon
ADDITIONAL FUNCTIONAL / ORGANIZATIONAL SUPPORT:
  • Assignments may be determined at a future date
QUALIFICATIONS AND ATTRIBUTES:
  • Strong organization skills with accurate attention to detail
  • Strong customer service focus, with exceptional poise and etiquette
  • Excellent communication skills and the ability to multi-task
  • Able to adapt and work in a fast-paced environment
  • Propensity to multi-task with excellent time management skills allowing the focus on results and the needs of the persons-served
  • Be a self-starter with a proactive attitude and anticipate the needs of the department and have a professional, friendly and enthusiastic attitude
  • High degree of technical savvy to navigate multiple systems, to include Microsoft Office Suite, NetSmart, Dayforce, and Foothold CM
  • Bilingual Spanish / English, a plus
  • Able to adapt and work in a fast-paced environment
EDUCATION AND EXPERIENCE:
  • HS Diploma or equivalency degree, or three-years, proven work experience related to clerical or office work
PHYSICAL CHARACTERISTICS:
  • Must be capable to sit or stand in front of a computer for long-periods of time
  • Able to work in open space floor plan
  • Capable of working patiently and respectfully with a diverse population
  • Work alongside co-workers within 3 feet
  • Must be able to move in tight spaces
  • Occasional lifting of >5 pound
