Job Overview:The Admission Coordinator is responsible for managing the admissions process for residents in a skilled nursing facility. This role requires strong organizational skills, excellent communication abilities, and a compassionate approach to working with patients and their families. The Admission Coordinator will act as a liaison between prospective residents, their families, and the facility’s clinical and administrative staff.

The Admission Coordinator will act as a liaison between prospective residents, their families, and the facility’s clinical and administrative staff.
Key Responsibilities:
- Admissions Process Management:
- Work directly with Accolade Healthcare Intake Department to process referrals timely. This would include screening prospective residents at local healthcare facilities, calling families and being a point of contact to answer outstanding clinical and financial questions.
- Conduct tours of the facility for prospective residents and their families, providing information about services and addressing concerns.
- Assessment and Documentation:
- Collaborate with healthcare professionals to assess the needs of prospective residents.
- Understanding of admission documentation and how to maintain medical records. Ability to prepare, explain and execute admission documents to residents and/or their families.
- Communication:
- Serve as the primary point of contact for families throughout the admissions process, providing regular updates and support.
- Coordinate with referral sources (hospitals, rehab centers, etc.) to facilitate timely admissions.
- Data Management:
- Must maintain logs and records to ensure accurate monthly data to all reporting teams.
- Analyze admission data and report on trends to help improve the admissions process and overall resident experience.
- Customer Service:
- Provide exceptional customer service to prospective residents and their families, addressing concerns and questions promptly.
- Foster a welcoming environment that promotes resident dignity and comfort.
- Associates degree in healthcare administration, social work, or a related field is preferred.
- Previous experience in admissions, customer service, or healthcare settings, especially in skilled nursing or long-term care, is highly desirable.
- Strong communication and interpersonal skills, with an ability to empathize with residents and families during the admission process.
- Excellent organizational and multitasking abilities, with attention to detail.
- Proficient in using Point Click Care, Microsoft Suite Applications, and Canva, is highly desirable
- Knowledge of healthcare regulations and compliance related to skilled nursing facilities, is highly desirable
- This position may require working occasional evenings and weekends to accommodate families’ schedules or special events.
