Job Description: Remote Life Health is seeking a dedicated and compassionate Patient Care Coordinator to join our team. The Patient Care Coordinator will be responsible for developing care plans, consulting with patients and family members to discuss health problems and educating patients about their condition and medication. The ideal candidate will have prior experience in patient care coordination and hold relevant certifications. As a Patient Care Coordinator at Remote Life Health, you will have the opportunity to work with a team of healthcare professionals and make a positive impact on patients' lives.

Responsibilities:

  • Phone Calls
  • Develop individualized care plans to address patients' personal health care needs
  • Consult with patients and family members to discuss their health problems and treatment options
  • Educate patients about their medical condition and medication, and provide specific instructions
  • Coordinate and communicate with healthcare professionals to ensure the best possible patient care
  • Maintain accurate and up-to-date patient records and documentation
  • Adhere to all relevant healthcare laws and regulations

Job Types: Full-time, Part-time

Pay: $16.00 - $18.00 per hour

Expected hours: 20 – 30 per week

Schedule:

  • 8 hour shift
  • Monday to Friday
