Submission Requirements

• Board Certified NP/PA - REQUIRED
• OH license (or willing to obtain) - REQUIRED
• DEA in hand/able to transfer - REQUIRED
• CSR/CDS in hand/able to transfer - REQUIRED
• Current - 3 years of the last 5 years - in Outpatient Family Medicine experience - REQUIRED (NO EXCEPTIONS)
• Date of agency's last communication with provider (put answer in "Additional Submission Details") - REQUIRED
