Job Description

The Care Manager is pivotal in providing care management services to beneficiaries with upcoming admissions to acute care hospitals or transitioning from other programs. The role involves systematic identification and addressing of fragmented care, tailoring interventions to fill gaps, and producing optimal clinical outcomes in a cost-effective manner. Responsibilities include pre-admission counseling, enforcing treatment plans during admission, coordinating care among different services, and following up post-discharge to ensure full recovery and compliance with prescribed care.

This is a fully remote position.

Work Environment

Remote work environment requiring strong WiFi and a private room. The role operates Monday-Friday, 8am - 8pm with shifts mostly falling 11:30-8pm EST
