As a Care Manager, you will develop, assess, and facilitate complex care management activities for members with primarily physical needs to deliver high-quality, cost-effective healthcare outcomes. This includes creating personalized care plans and educating members and their families. You will also coordinate care between members, their families/caregivers, and healthcare providers, ensuring person-centered care. Your role involves ongoing follow-up, monitoring of member status, and adjustments to care plans as needed. Additionally, you will provide resource support and facilitate care management to ensure members have timely access to required services. The role requires collecting and maintaining member information and care management activities in compliance with regulations. You will also educate members and their families on disease processes, care options, and healthcare benefits, alongside providing feedback to leadership on care and quality delivery improvements.
