Job Description

Seeking a dedicated and skilled professional to perform care management duties to assess, plan, and coordinate all aspects of medical and supporting services for select members. This role focuses on promoting quality, cost-effective care across the continuum of care. Responsibilities include developing and adjusting care plans, coordinating services among healthcare providers, providing patient and provider education, facilitating access to community-based services, and maintaining pertinent clinical information in medical management systems.

Hard Skills

• RN License (compact)
• Telephonic Case Management experience
• Associate's Degree
• Microsoft Office Suite experience

Soft Skills

• Ability to assess and adjust care plans
• Strong coordination and communication skills
• Ability to educate patients and providers
• Effective monitoring and facilitation
