Immediate Hiring for Remote RN Case Manager in Florida & Michigan
Job Description:
- Provide support to medical, behavioral, and social support staff to determine needs, eligibility, and services for members.
- Conduct screening and risk assessment interventions per program guidelines.
- Educate providers and community resources on program components and available support services.
- Assist with modification and management of care plans
Qualifications:
- 3 years of experience with clinical nursing experience in home health, hospice, managed care or case management is Mandatory
- Active RN Compact license in the state of Florida & Michigan is Mandatory
