***MUST LIVE IN LAURENS OR CLINTON, SC TO APPLY***

QUALIFICATION

• Associate’s or Bachelor’s Degree from an accredited nursing program
• Licensed to practice nursing by the State of South Carolina
• One year experience in public health, hospital, or long-term care nursing
• Have strong record-keeping and documentation skills
• Able to pass drug test, criminal background check, and others deemed by SC state
• Have valid SC Driver’s License and able to drive without restrictions
• Home health care experience is a plus but not required

RESPONSIBILITIES

• Contact clients’ physicians to obtain direct care needs
• To assess clients age 18 - up in their homes and/or living facilities for non-skilled needs and generate plans of care for caregivers (Complete Service Authorizations)
• Visit clients, provide re-assessments of clients for continuation of care, and submit required documents “timely” to government agencies
• Supervise, train, review, and sign caregivers’ documentations
• Train caregivers/staff members basic care giving techniques and disease management
• Expand knowledge and capabilities by attending educational workshops, conferences, and etc.
• Other duties as assigned by management
