Adara Home Health is Hiring a Transitional Care Manager Near Buffalo, MN

Our Mission: Honoring People Our Values: Integrity, Compassion, Curiosity, and Helpfulness The TCM assumes general responsibility for developing and implementing sales and marketing strategies which include customer and market research, marketing plan development and implementation, community education, growth, and public relations of the branch and territory defined by management. Responsibilities also include initial and periodic assessment of clients, as assigned. The position reports to the Area Manager. SCHEDULE Full Time COVERAGE AREA Buffalo and surrounding cities. BENEFITS Paid Time OffPaid HolidaysRecognition ProgramEmployee Referral Program401k - 100 % Vested Upon EligibilityMedical, Dental, Vision, Life, AD&D, Pet insuranceMobile provider discountsTuition Reimbursement MAJOR RESPONSIBILITIES Community Relations:Develops and implements sales and marketing plan for all company product and service offerings, consistent with market analysis.Identifies and recommends development of new products, services, and niche markets and offerings as well as current product and service enhancements.Ensures information is up-to-date on competitors including pricing, census, product information, and marketing strategies.Contacts key physicians and other health care representatives on a regular basis.Furnishes educational information on Adara services by various communication venues that include and are not limited to, letter, telephone, or in person to the community.Makes presentations to groups, explaining all Adara services.Maintains daily log of educational calls, in services, and community events attended.Develops and maintains relationships with referral sources, including large hospitals and health care systems, major physician practices, managed care organizations, third party administrators (TPA), and other potential large referral sources.Prepares special market studies and research as needed.Assists in the development of company marketing materials.Expands awareness of Adara services to professional health care entities.Client Care:When needed, performs initial and periodic assessment of clients to determine needs and effectiveness of care.Participates in the after-hours on-call process, as requested.Maintains confidentiality of all information pertaining to clients, families, employees and referral sources.Participates in case conferences, as requested.Education/Teaching:Maintains knowledge of home care industry trends and educates branch personnel.Educates, trains and orients branch personnel in customer relation skills and techniques.Educates referral sources on home care services and the referral process.Leadership:Provides role modeling of teamwork, accountability and a positive attitude.Demonstrates and encourages appropriate problem solving methods and willingly listens and responds to branch personnel concerns.Instills confidence in branch personnel regarding customer relation skills.Demonstrates self-direction to prioritize and accomplish job responsibilities.Understands and participates in a team approach and concept.Financial:Ensures growth and profitability of the company through the responsible use of company resources and educating the community to our services.Assists in the initial and periodic assessment of clients including payer authorization and requirements.Applies basic knowledge of financial resources for home care clients when educating referral sources.PHYSICAL/ENVIRONMENTAL DEMANDSMedium to Very Heavy. Moderate lifting necessary of marketing equipment and materials. Responsible for client care needs which may require transfers and lifting of clients. Nursing care provided in individual client homes. Must be adaptable to a variety of environments and community settings. Must be able to drive or use public transportation in all types of weather.Exerting 20-100 pounds of force occasionally, and 25 to 50 pounds of force frequently, and 10 to 20 pounds of force constantly to move objects. Physical demand requirements are in excess of those for light to medium work.QUALIFICATIONSCurrently licensed as a R.N. in the state of Minnesota or other health care professional degree required. The license must not have been revoked, suspended and without limitations or restrictions.Have at least two (2) years experience in a health care facility or home care field.Strong organizational skills and the ability to work independently.Have U.S. Citizenship or evidence of valid Alien Work Permit.Demonstrates ability to make appropriate clinical judgements.Excellent written and oral communication skills; interpersonal communication and teaching skills; public speaking skills.Basic computer skills and the ability to learn new software. Knowledge of Work and Excel a plus.Ability to manage a variety of assignments while establishing priorities and assuring that necessary duties are completed.Proof of negative Tuberculosis symptom blood test.Current CPR certification recommended.Access to a dependable vehicle or public transportation in order to travel to multiple company business stops a day.A valid driver’s license and proof of car insurance when using a personal vehicle for company business.Disclose any conviction and criminal history records pertaining to any crime related to the provision of health services. A candidate who has been convicted of a crime will not be hired.Pass initial and ongoing background studies and screenings including but not limited to those of the Minnesota Department of Health and the Federal Office of the Inspector General’s List of Excluded Individuals and Entities.
