Mobile Recovery Nurse Care Coordinator (LPN, RN)

Position Overview:
Join our team where you will play a pivotal role in reducing stigma and expanding access to substance use disorder treatment in underserved and under-resourced rural communities. Collaborating closely with the Mobile Recovery Resource Specialist to provide holist wraparound services under the oversight of the Mobile Program Manager, you'll ensure the smooth coordination and delivery of vital care to our clients. Essential Functions:
· Collaborate with the Mobile Program Manager to assess, coordinate, and fulfill the needs of rural community clients.
· Conduct intake screenings, review and complete necessary consents and health histories with patients.
· Complete documentation, progress notes, and assist in gathering medical, behavioral health, and pharmaceutical records.
· Perform vital signs, urine drug screens, urine pregnancy tests, IM and SQ injections.
· Manage appointments, verify insurance, and perform administrative tasks.
· Ensure the cleanliness and functionality of the Mobile Unit and assist in interoffice communication with other departments.
· Ability to drive the unit to locations of service.
· Adhere to strict confidentiality protocols for client information
Availability:
· Minimum of 40 hours per week,
· Monday- Friday 8 a.m.- 5 p.m.

Qualifications, Education, and Experience:
· Current Licensed LPN or RN in the State of Michigan
· 2 years of Managed Care experience (preferred)
· Experience in substance abuse treatment (preferred).
Skills Needed:
· Excellent interpersonal and communication skills.
· Strong computer proficiency and data entry skills.
· Competency in working with co-occurring populations.
· Effective team interaction, critical thinking, decision-making, and problem-solving skills.
· Time management and multitasking abilities.
· Experience with EMR systems (preferred).
· Ability to de-escalate stressful situations.
· Ability to sit and/or stand for long periods, lift up to 50 lbs
Required Trainings/Certifications:
· Valid Driver’s License
· Blood-borne Pathogens training
· CPR/First Aid certification

Salary : $31 - $33
