Job Summary:

• Assisting clients in receiving services by telephone and in person.
• Getting clients involved in beneficial activities.
• Assessing clients and gathering relevant information.
• Offering information and supporting clients and their families.
• Contacting and making referrals to other agencies and services.
• Maintaining accurate records and preparing reports.
• Participating in training, supervision, and meetings.
• Watch for signs of child abuse.
• Provide crisis intervention.

Shift: Day 5x8-Hour (08:00 - 16:30)

Job Type: Contract (13 weeks)

Pay Range: $50/hr - $ 56/hr (Depend on experience)

Experience required:

• 2 years of experience required
• First-time travelers accepted (Y/N): No
• Locals accepted (Y/N): Yes

Certifications required:

• ACLS, BLS

Description: Looking for Case Management Social Worker with experience in both Social Work and Discharge Planning.
Experience with Allscripts/CarePort/Wellsky preferred

Pay: $50.00 - $56.00 per hour

Expected hours: 40 per week
