Aditi Consulting is Hiring a Clinical Support Coordinator Associate Near Long Beach, CA
Summary:The Clinical Access Program’s Initial Health Appointment Outreach Team ensures that all newly enrolled Medi-Cal members have an IHA visit with their PCP within 120 days of enrollment. The Clinical Services Coordinator will report to the Clinical Training and Auditing Supervisor. In this role you will be responsible for making outreach calls to Medi-Cal members on a daily basis and maintaining documentation of this outreach.

Responsibilities:
- Perform daily outreach to Medi-Cal members to help coordinate Initial Health Appointments
- Be responsible with handling Personal Health Information (PHI)
- Document calls and call outcomes in spreadsheets and/or dashboards
- Research member eligibility in Sales Force (Care Connect) and FACETS
- Assist with regulatory audits
- Interact with provider offices for both Los Angeles and San Diego counties
- Participate in Team and Department meetings
- Perform other Duties as assigned to meet Department needs
- Requires at least 3 years of prior relevant experience
- Bilingual Spanish/English
- Expertise in Microsoft Office (Word, Excel, Outlook), Sales Force (Care Connect), FACETS
- Capable of working independently and as part of a team
- Excellent written and verbal communication skills and professional phone etiquette
- Health Plan, IPA, Public Health, Quality Improvement experience desired
