Social Worker wanted for an Adult day care and MUST be a Licensed Social worker or Certified Social worker to apply. New Grads with lsw or csw are welcomed to apply.

Duties include:

• Assists clients to complete Medicaid and SNAP applications.
• Compiles social history and conducts a psycho-social assessment including formal and informal support systems, mental and emotional status, community and financial resources, and care giver data.
• Provides discharge planning and assists in the transition and follow-up as appropriate.
• Utilizes community resources to assist client if not provided by the Center and work with these agencies to coordinate services.
• Contributes to a positive work environment and overall team effort.
• Performs related duties and responsibilities as required or directed by the administrator.
• This is not a clinical position.
• Applicants without a Social work license or certification will not be considered.
