JOB SUMMARY Carries out basic patient care under the supervision of a registered nurse in accordance with the standards of care for the center. Demonstrates the appropriate skills and ability, based on level of training and scope of practice, to care for patients based on the age of the patient. Age groups may include the infant, toddler, school age/adolescent, adult and older adult. DUTIES AND RESPONSIBILITIES
Provides direct patient care for assigned patients under the direct supervision of a licensed nurse.
Obtain pre and post treatment vitals, weight and vascular access condition. Report any unusual changes or findings to the nurse.
Responsible for timely and accurate documentation of information related to patient treatment including vital signs, treatment parameters, and change in condition.
Assist in the maintenance of a safe and clean working environment.
Participates in collaborative identification and reporting of patient safety issues and ensures clean and safe patient environment; seeks assistance with removal of any potential safety hazard from room and reports findings to nurse or charge nurse
Answer call lights and unit phone to assist with questions and concerns from patients and family members.
Communicate, monitor and document pertinent patient information
Assist with admission, transfers, transporting, lifting, and discharge of patients
Make rounds and respond promptly to patients, families, and clinical team requests.
Work effectively and collaboratively in a diverse and multi-cultural environment.
Assist in organization, cleaning, and safety of the unit and patient rooms
Qualifications REQUIREMENTS
High School Diploma or GED
1 YRS Direct Patient Care
Experience working in an Acute Care Facility
EDUCATION
High School Diploma or GED
Graduate of an Accredited C'NA/Medical Assistant Program
Graduate of a Patient Care Tech (PCT) Program
CERTIFICATION, LICENSURE
Current CNA License
BLS, required.
CPR within 30 days of hire
KNOWLEDGE SKILLS & ABILITIES
Ability to meet the emotional and spiritual needs of patients and families.
Be able to carry out the plan of care by performing/assigning delegated tasks.
Complete nursing assessments within required time frames, and in accordance with policy and procedure.
Identify and act on predictable needs of the patient.
Ability to meet the health care needs of patient by assessing and assisting the patient to meet expressed needs.
