Advantage Case Management provides case management for persons with disabilities through Medicaid Waiver programs. We offer case management services in the Seven Counties, Communicare, Lifeskills, River Valley, Pennyroyal, Bluegrass and Adanta regions.

Job Description:

Louisville Area-Full Time

Case managers assess and reassess the medical, vocational, psychosocial and economic needs of people served to coordinate services as appropriate and/or approved, ensure cost-effectiveness and satisfaction with service delivery and facilitate communication between all parties involved. Case managers monitor the implementation of person-centered services both waiver and non-waiver and document all activities through written reports.

Compliance administration includes file reviews and management, document organization and interaction with internal staff and managers.

This job is for an Entry-Level Case Manager starting with a small caseload and compliance/administration duties while learning the job and slowly increasing case management duties.

ESSENTIAL JOB FUNCTIONS

Conducts or participates in and documents comprehensive assessments of participant and/or participant needs via state tools. Leads development of participant's person-centered service plan ensuring it is consistent with assessments. Coordinates/refers for delivery of needed services as identified in the assessment. Monitors participant progress and satisfaction by completing referrals, noting what is important to/for the participant, tracking participant appointments and services, following up on needs, services and appointments in order to provide next-steps and coordination. Performs advocacy on behalf of participant with providers, family, guardians, state agencies, and other entities and facilitates the participant to advocate on their own behalf whenever possible. Creates and maintains necessary participant documents as required by Advantage Case Management and state agencies including certification documents, recertification documents, plans of care, employment documentation, case management documentation and others. Follows guidelines for services as set by regulation and Advantage policy. Communicates with supervisors about case management as a whole including participant status. Provides follow-through and follow-up for the participant for all issues. Ensures health, safety and welfare.

EDUCATION

  • Bachelor’s degree in Social Work, Psychology, Sociology, Human Services, or related field as approved by Medicaid, or as required by applicable funding source.*
  • Completion of state-required case management/service coordination course within six months of date of employment.

EXPERIENCE

  • Minimum of two years experience* in working with people in a relevant or related field or position
  • Good interpersonal skills and written and oral communication skills
  • Experience with waiver documents and regulatory compliance preferred but not required.

PHYSICAL DEMANDS/REQUIREMENTS

  • Reliable transportation for frequent travel.

  • Effective "person-first" communication and language skills or willingness to learn and implement
  • Ability to prioritize tasks
  • Works independently after training with strong organization and time management skills
  • Manages changing workflow as participant's needs/crisis situations change or occur
  • Facilitates Service Team meetings
  • Provides services that are within the scope of definition of case management
