Agape Health Systems, Inc. is Hiring a Family Medicine Nurse Practitioner Near Baltimore, MD

Agape Health Systems, Inc. is a primary care practice (a subsidiary of Hope Health Systems, Inc.) offering quality and compassionate health care to infants, children, adolescents and adults. We are dedicated to establishing long-term relationships with our families to effectively provide acute care, preventative medicine, parent education and more to insure patients receives every benefit possible. We are seeking Full Time and Part Time Staff to join our Team

Job Summary: Responsible for providing patient care as appropriate in the clinical setting, for medical case management, coordination of services with other agencies, clinical tracking of special population groups and any other activities or duties required to assure optimal patient services. Pediatrics experience a must.

Essential Functions

  • Provides comprehensive physical assessment of patients and evaluates the outcome of patient care.
  • Prescribes or recommends drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures.
  • Arranges referrals, consultations, therapeutic services, and confers with other specialist on course of care and treatment.
  • Establish medical diagnosis for common short-term and chronic stable health problems.
  • Order, perform, and interpret laboratory and diagnostic tests.
  • Order and perform diagnostic, therapeutic, and corrective measures. Prescribe drugs
  • Provide urgent care.
  • Refer patients to appropriate licensed physicians or other health care providers as needed.
  • May refer patients to physician for consultation or to specialized health resources for treatment.
  • Provides direct patient care as needed and appropriate to serve the community in accordance with the organization’s Health Care Plan.
  • Provides appropriate documentation of clinical care provided in a timely manner. This includes maintenance of patient charts in accordance with clinic protocols, and timely documentation of clinical procedures on encounter forms.
  • Complies with regulations regarding consultations and other services as required by patients’ insurance coverage.
  • Performs timely entry into, and manipulation of, the Electronic Health Record.
  • Assumes fiscal responsibility with working knowledge of evidence based medical model.
  • Effectively uses resources of the patient and the institution.
  • Is aware of and adheres to specific capitations and limitations of the patients insurances while organizing and optimizing individualized care based on the disease state and restraints imposed by the HMO or insurances.
  • Utilizes referrals to Home Health/Behavioral Health in order to provide optimum care in the appropriate setting.
  • Has knowledge of and utilizes all available resources in the event that the patient's resources are limited or unavailable in order to provide medical care and social well-being.
  • Able to use current EHR - Credible

Nonessential Functions:

  • Provides clinical leadership and acts as a resource to other members of the health care team, patients, or other members of the community.
  • Provides clinical supervision and acts as a resource to nurse practitioners working in the clinic as appropriate.
  • Works with center manager to facilitate smooth operation of clinic flow.
  • Assures quality of patient care by participating in the organization’s Performance Improvement activities.
  • Cooperates with compliance review activities.
  • When needed, assists the Safety Officer with employee health activities, especially in the event of an exposure to bloodborne pathogens.
  • Participates in team/staff meetings; attends in-service meetings and applicable workshops.
  • Performs any and all other duties required to provide for optimal patient care.
