Case Managers assess needs, coordinate, and monitor community services and benefits for individuals over 60 or individuals under 60 with disabilities and enrolled in Choices for Care to promote health, independence, social and economic wellbeing. This is a field position of some complexity, with particular attention paid to the most needy and at-risk individuals in the community at large.

The successful candidate will have a Bachelor’s Degree and some experience is preferred.
