What are the responsibilities and job description for the Specialized Case Manager position at Age Well?
Specialized Case Managers provide intensive care coordination to older adults who are vulnerable or at risk, or who have medical, social, or emotional challenges that add complexity to their care needs. In order to meet the needs of these older adults, the Specialized Case Manager needs to have and maintain an understanding of our community's diverse resources, services and benefits for people over 60 in order to promote health, independence, social and economic wellbeing. The Specialized Case Manager visits clients in the community to offer practical assistance, person-centered guidance, and support. This is a field service position with a degree of complexity and challenge.
The successful candidate will have a Bachelor’s Degree (BSW or MSW preferred), and three years experience in social services. Candidates with experience in supporting individuals with behavioral health needs are preferred.
