Characteristicsof Position:

This is a professional clinical position and member of amultidisciplinary team which assumes the responsibility for providing comprehensive,community-based treatment and support services to individuals experiencingchronic homelessness due to factors including substance abuse, mental illness, and/orchronic physical illness.

Core Responsibilities/Essential Functions:

• Performs psychosocial assessments, treatmentplanning/monitoring and discharge planning.

• Provides individual, group, and family counselingto individuals requiring complex interventions including but not limited to homelessness, substance abuse, mental illness, developmentaldisabilities, and/or chronic physical illness.

• Organizes support groups or counsel familymembers to assist them in understanding, dealing with, and supporting theclient

• Acts as consumer advocate in order to coordinaterequired services or to resolve crisis

• Performs commitment determinations and/orexecute orders of involuntary commitments.

• Provides case management services related toidentified treatment needs

• Refers consumer or family to community resourcesto assist in recovery from mental or physical illness and to provide access toservices such as financial assistance, legal aid, housing, job placement oreducation

• Monitors, evaluates, and records client progressaccording to measurable goals described in

  treatment and care plan

• Adheres to regulatory bodies and licensingrules, guidelines and established protocols to carry out all duties required bycontract, licensure, accreditation, and certification relative to agency,program, service assignment and agency designated models of treatment.

• Provide clinical supervision to other members oftreatment team.

• Participatesin "on-call" rotation for afterhours responsibilities

• Assistwith the development of therapeutic group activities; assist with theimplementation of group activities at the Drop-In Center.

• Providetraining and learning opportunities for staff on Best Practices and othertherapeutic techniques.

• Responsiblefor review of assessment, treatment plans, and other documentation; providefeedback to staff and facilitate improvement as needed.

• Facilitatetreatment team meetings and client staffing. Document meeting minutes, identify strategies, action plans and followup.

• AssistProgram Supervisor as needed.

• Performs related work as required /assigned.

Pre-requisite Requirements:

• Master's degree in social work, counseling,psychology or other human services field.

• Current State of Florida License in MentalHealth Counseling, Social Work, or a related human services field.

Supervision: This position reports to the Team Leaderand provides clinical supervision to other non-licensed team members.
