Duties: Promote HIV/AIDS health care services within the Miami community by assisting individuals to adopt risk reduction behavior. Identify and contact members of high-risk groups. Act as the liaison for AHF and promote programs that improve individual and overall community health. Participate in collecting data to identify community health needs. Conduct patient assessment and care planning. Refer community members to needed health services. Develop treatment plans based on the acuity determination from the initial health and needs assessment in concert with the Ryan White client, the medical providers, client identified family/significant other and other community resources. Review medical records to monitor HIV/AIDS Bureau measures and other preventative measures such as A1C values and assist clients in reaching or maintaining acceptable levels. Communicate with patients to ensure they have access and knowledge of their health conditions, community benefits, healthcare resources, treatment plans, and medication adherence. Provide treatment adherence counseling to ensure readiness for and adherence to complex HIV treatments, and to ensure an understanding of the importance of compliance with medical appointments for monitoring. Participate in the development of Medical Case Management policies and procedures, and ensure ongoing compliance. Maintain member identifiable health information in a confidential manner and in accordance with AHF policy and HIPAA regulations. Participate in the Quality Improvement program by collecting, analyzing, and reporting on QI indicators on a routine basis. Provide cultural mediation related to health services or information for community members.
Requirements: Two (2) years of community health care, healthcare administration, or related experience: working with HIV related programs; assisting clients with HIV/AIDS clinic intake paperwork; assisting clients with forms for medical eligibility, diagnosis, and financial screening; assisting clients with benefit paperwork; conducting initial interviews to determine client readiness in seeking HIV specialty care; ensuring clients are accurately and appropriately scheduled for medical appointments; communicating with clients to remind them of their medical appointments; promoting and explaining the HIV programs available in the community; and promoting regularly scheduled outpatient HIV case services such as client education, clinical intervention, and medication protocols.
Salary: $52,993.72/year
