Responsibilities

Do you believe housing is healthcare and we all deserve a safe home? Do you want to be creative and give people support in obtaining and maintaining a home? Do you enjoy building relationships with community landlords and housing providers? Do you want to spend your days crossing the T’s and Dotting the eyes to make certain those we serve our housed? This may be your role -

In addition to other responsibilities, this position will have the opportunity to:

  • Assess Family needs, strengths, and abilities to build a collaborative action plan to reach goals of increase independence
  • Develop budget with clients to determine housing cost and to maintain housing
  • Provide hand-on life skills teaching and coaching including housekeeping, budgeting and meal preparation to ensure ability to maintain housing
  • Conduct home visits to assess and provide supports to establish a hygienic living environment to meet Housing Quality Standards (HQS)
  • Provides creative interventions and linkages to support clients in meeting housing and health goals.
  • Locates affordable housing options
  • Meets with internal and external interdisciplinary treatment team members to coordinate client services and documents outcomes of meetings
  • Provide information and negotiates with landlord and prepares for informal hearing/appeals.
  • Submits all documentation in an accurate and timely manner, including electronic records.
  • Other duties as assigned.

Skills/ Requirements

  • Minimum of two years' experience in case management which may include internships, preferred
  • Substance abuse treatment or residential setting experience preferred.
  • HIV/AIDS knowledge preferred
  • Proficient computer skills.
  • Knowledge of Substance Abuse and Mental Health Symptoms and interventions
  • Must have a bachelor’s degree in Social Work, Health or Human Services from an accredited university or equivalent experience
  • Ability to work independently with minimal direction
  • Effective verbal and written communication skills
  • Bilingual preferred.
  • HIV 500
  • Valid Florida Driver's license and ongoing proof of insurance required and ability to provide own transportation.
