- Provide quality healthcare to each patient in adherence with all applicable laws, regulations, and policies
- Focus on the medical needs and treatment of patients according to an active plan of care
- Assess and chart observations of the patient's condition at each visit
- Conduct Patient evaluation tasks including reviewing medication and vital signs
- Administration of prescribed medication, treatments, and therapies
- Coordination of care with physicians, PT and other individuals in the patient's care plan
- Provide education to patients, caregivers and family members
- Active RN/LPN License to practice in the State of Florida, and
- One year of clinical-based experience with specialty of home health preferred.
- Current CPR certification
- Self-driven and accountable to provide excellent patient care and attention to detail
- Ability to deliver skilled services such as assessment and skilled observation, wound care and dressing changes, injection administration, medication administration, patient and caregiver education
- Maintain a level of knowledge to navigate information through an electronic medical records system
- As a condition of employment, must successfully complete drug test and background check including criminal history check and character references
