Albany Medical Center is Hiring a Case Manager HIV Near Albany, NY

Substance Abuse Specialist CASAC - required
Minimum of a bachelor's degree in human service profession- required
Master's degree in counseling, psychology, or related field - preferred
Certification as a CASAC (certified Alcohol/substance abuse counselor) - preferred
Minimum of three years substance abuse experience (preferably with at last one year of this experience in a hospital or clinic setting) - required
Previous Experience With HIV-infected Population - Preferred.NYS Department of Health certification in HIV counseling and testing - preferred
Ability to work in a fast-paced, interdisciplinary team setting - required
Extensive experience as a CASAC providing substance abuse counseling services may be substituted for bachelor’s degree.
RESPONSIBILITIES:
Under the direction of the Director of Client Services for the AIDS Program and consistent with NYS Department of Health standards for an AIDS-Designated Center, the substance abuse specialist is responsible for:
  • Substance Abuse Counseling: Provides individual and/or group counseling to AIDS Program patients regarding relapse prevention, harm reduction, and other substance abuse related topics. Groups are run at times mutually agreed upon in supervision and will be held either at the clinic or other location, as appropriate.
  • Consults: Provides alcohol/substance abuse related consults that may be requested by inpatient staff (on a limited basis) as well as outpatient providers and general case managers.
  • Patient Education: Provides individual and group instruction about addiction, recovery and treatment options, substance abuse and relationship to immune system, risk reduction education, 12 step programs, community referrals for treatment and available support mechanisms.
  • Staff Education: Provides periodic updates and information to the AIDS Program interdisciplinary team regarding the clinical manifestations of substance abuse and addiction, particularly as it relates to HIV disease. Staff education programs are both formal and informal and include but are not limited to nursing staff, case management, physicians and other clinical staff, as necessary.
  • Assessment: Based on referrals from case management, medical providers or patient self-referrals, completes a brief assessment of patient's drug abuse and alcohol history, desire for recovery (if currently using), relapse potential (if currently in recovery), supportive environment, and other psychosocial issues as they pertain to patient's substance abuse potential. Completes comprehensive assessment for patients being referred for rehab. [Note: Patients being referred would ordinarily include those who are actively using and want treatment, are at risk of relapse, are agreeable to referral, and are newly in recovery (less than 1 year) and need additional referrals/support.]
  • Treatment Plan: Establishes treatment plan for all patients seen for ongoing substance abuse counseling and initiates treatment plan upon the completion of brief assessment. Treatment plans are updated on a periodic basis. Assesses progress towards goals and objectives identified in the treatment plan.
  • Referrals: Based on individual assessments and treatment plans, refers willing patients to inpatient and/or outpatient substance abuse or MICA programs, as appropriate. Provides intensive follow-up monitoring and input to assist patients in substance abuse programs, as needed by the patient and the provider.
  • Crisis Intervention: On a limited basis, the substance abuse specialist assists clinical staff when patients are in psychosocial crisis.
  • Linkage Mechanisms/ Outreach/ Prevention: The substance abuse specialist establishes linkages with community-based social, mental health, and substance abuse service providers, particularly as they relate to substance abuse treatment and referral. Working with these other agencies, the substance abuse specialist establishes mutually agreed upon referral mechanisms designed to facilitate treatment in an efficient way and protect patient confidentiality. The substance abuse specialist participates in outreach and prevention initiatives as appropriate with our specific population served.
  • Program Development: Provides input regarding alcohol/substance abuse issues and concerns to medical director, manager of client services, and other staff involved in strategic planning, policy development and implementation, and program development. Coordinates patient satisfaction surveys for substance abuse services.
  • Care Coordination: Works as part of the interdisciplinary team, collaborating with external and internal case managers and medical providers. The substance abuse specialist participates in care coordination meetings, giving input on patients in substance abuse specialist’s caseload in addition to acting as consultant for patients with potential addiction issues.
  • Documentation: Documents patient interactions in the electronic health record and documents corresponding provision of services in the AIRS (AIDS Institute Reporting System) data base.
Language skills:Ability to read and understand medical information; knowledge of medical terminology helpful. Fluency in written and oral English. Spanish is a plus. Ability to respond to routine inquiries or complaints from clients, regulatory agencies, or members of the community.
Mathematical Skills:
Ability to calculate figures and amounts such as discounts, interest, proportions, and percentages. Basic computer skills and ability to type are preferred.
Reasoning Ability:Ability to define problems; collection of data; and develop a plan of action.
