What you will do in this role:

• Assumes responsibility and accountability for the care of those residents treated by SLP/CFY if applicable.
• Assesses resident’s needs; identifies and initiates appropriate speech therapy interventions; collaborates cooperatively with other patient team personnel in maintaining standards for professional speech therapy practice and interdisciplinary teamwork.
• Observes and evaluates treatment affect, accomplishment of short- and long-term goals. Recommends change to physician, if needed
• Performs an appropriate evaluation on each patient and performs reassessments, as per policy. Completes accurate and timely documentation. The population is adult and geriatric residents in an SNF setting.
• Able to assess resident pain interfering with optimal level of function of participation in rehabilitation; makes appropriate interventions to reduce pain; makes appropriate physician contact for intervention.
• Formulates and updates resident specific plan of care for speech therapy.
• Other duties as assigned.

Qualifications

• Current Florida licensure as a Speech Language Pathologist
• Must have long term care experience. Knowledge of PDPM, Swallowing, and LTC program development.
• Familiar with Net Health and Point Click Care

Salary range: $39.00 to 45.00 per hour. We would expect those hired for the job to have a salary within his hourly rate, however, certain circumstances may allow us to pay outside of this hourly rate.
