All's Well is Hiring a Hospice LVN Near Beaumont, CA

As a Hospice LVN, you will provide nursing care to patients in their homes or hospice facilities. You will work closely with the interdisciplinary team to manage symptoms, provide emotional support, and ensure the highest quality of care. Your role is crucial in helping patients achieve comfort and dignity at the end of life.
8 hours shifts Per diem and On call available
Key Responsibilities:
Provide direct nursing care, including administering medications, monitoring vital signs, and performing wound care.
Implement and follow individualized care plans in collaboration with the interdisciplinary team.
Assist with activities of daily living to promote patient comfort.
Monitor and manage symptoms such as pain, nausea, and respiratory distress.
Adjust care plans based on patient needs and communicate changes to the health care team.
Offer emotional and psychological support to patients and their families.
Educate families about the dying process and what to expect.
Maintain accurate and up-to-date medical records, including care plans, progress notes, and medication administration records.
Ensure compliance with hospice policies and regulatory requirements.
Communicate effectively with patients, families, and the interdisciplinary team.
Participate in team meetings and case conferences to discuss patient care.
Qualifications:
Current LVN license in the state of CA.
Minimum of 1 year of nursing experience, preferably in hospice or palliative care.
Compassionate and empathetic with excellent communication and interpersonal skills.
