Position Title: Independent Living Services (ILS) Coordinator

Employment Type: Part-time- Full-time (12-40 hours per week)

Work Arrangement: Hybrid - Primarily conducted at individuals' homes or within the community

Hours: Flexible, based on caseload demands

Caseload: 3-4 individuals

Compensation:
$21-22 an hr - based on experience
Mileage reimbursement at $0.67 per mile

Requirements:

• Current TB test within the last two years
• CPR and First Aid certification
• Ability to pass Live Scan background check
• Authorization to work in the United States
• Valid driver's license
• Access to a reliable vehicle
• Car insurance
• Minimum of 1-year experience working with individuals with developmental disabilities

Responsibilities:

• Supervise and coordinate services for individuals served, ensuring their needs are met
• Accompany individuals to medical appointments
• Facilitate weekly planning sessions
• Develop comprehensive lesson plans with Specific, Measurable, Achievable, Relevant, and Time-bound (S.M.A.R.T.) goals
• Assist individuals in developing and managing life skills
• Provide transportation for individuals to community outings and appointments
