- 1/15 start date, 13-week assignment, Mon-Fri schedule. Candidate will split between Sheridan and Salem Transitional Care. Sheridan is a 51-bed SNF offering short and long-term care, respite and rehab; Salem is an 80-bed facility. Required Qualifications PT (skilled)
