• Age/Population of Patients AgesESSENTIAL FUNCTIONS A.Evaluates plans, directs, supervises and conducts habilitative and rehabilitative programs for patients with physical limitations.B.Consults and communicates with patient, family, and other multi-disciplinary team members and service providers.C.Records and reports patient s progress towards established goals, and data related to care in the patient s medical record/chart.D.Instructs patients in exercises, ambulation, and activities of daily living, safe body mechanics, home exercise programs, and return to work precautions.E.Assists in maintaining the department and equipment in a clean and orderly manner in accordance with department policy regarding infection control and preventive maintenance.F.Attends and participates in department in-services and meetings.G.Delegates and supervises activities of physical therapy assistants and technicians, to assure efficient use of department resourcesH.Is actively involved in planning and promoting department growth and program developmentI.Assists in cost containment through efficient use of work time and supplies. • Graduate of Physical Therapy Program.
