The PT will be responsible for conducting evaluations and developing treatment plans for patients needing skilled services, such as physical therapy and rehabilitation. This role requires a compassionate professional dedicated to improving patients' quality of life through personalized therapy programs.Key comprehensive evaluations of patients to determine their physical therapy needs.Develop and implement individualized treatment plans aimed at achieving the highest functional outcomes for patients.Provide skilled physical therapy services, including but not limited to therapeutic exercises, manual therapy, and functional training.Monitor and document patient progress, adjusting treatment plans as necessary to achieve optimal results.Maintain accurate and timely documentation in compliance with regulatory and facility requirements.Participate in facility meetings, in-services, and continuing education opportunities to stay current with best practices.
