Allserve is Hiring a Registered Nurse (RN) Near Latrobe, PA

QUALIFICATIONS:

· Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be Graduate of an approved RN program.

· Must have a minimum, one (1) year of experience as a supervisor in a hospital, nursing care facility, or other related health care facility.

· Must have training in rehabilitative and restorative nursing practices.

· Must possess a current, unencumbered, active license to practice as an RN in this state.

· Must demonstrate knowledge and skills necessary to provide care appropriate to the

age-related needs of the residents served.

· Must possess a valid CPR certification

· Must have effective oral and written communication skills

· Must meet all local health regulations and criminal background checks, post-offer drug trust, and post-off physical

· Must maintain all required licenses, certifications, and continuing education requirements by the state

· Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.

Resident Rights

· Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.

· Ensure that you treat all residents fairly, and with kindness, dignity, and respect

· Ensure that all nursing is provided in privacy

· Knock before entering the resident’s room

· Report all grievances and complaints made by the resident to the Nurse Supervisor/Licensed Practical Nurse

· Report all allegations of resident abuse and/or misappropriation of residents property immediately to the Nurse Supervisor

· Honor the resident’s refusal of treatments request. Report such requests to your supervisor

· Communicate respectfully and appropriately, respect culture differences among residents and staff

ESSENTIAL RESPONSIBILITIES AND DUTIES:

· Assist the Director of Nursing in directing the day-to-day functions of the nursing activities in accordance with current rules, regulations, and guidelines that govern the long-term care facility.

· Make written and oral reports/recommendations to the Director as necessary/required, concerning the operation of the nursing service department.

· Ensure that all nursing service personnel are in compliance with their respective job requirements.

· Participate in facility surveys (inspections) made by authorized government agencies as requested by the Administrator or Director of Nursing.

· Admit, transfer, and discharge residents as required.

· Complete accident/incident reports as necessary.

· Interpret the department’s policies and procedures to personnel, residents, visitors, and government agencies as required.

· Perform administrative duties such as completing medical forms, reports, charting in Point Click Care, etc.

· Agree to not disclose username and passwords for accessing resident/facility information and promptly report suspected or known violations of such disclosure to the Administrator.

· Agree to not disclose residents protected health information.

Personnel Functions

· Ensure that a sufficient number of licensed practical nurses, certified nursing assistants, and/or registered nurses are available at the start of your shift.

· Assign sections and/or in completing and performing such tasks

· Make daily rounds of the nursing department to ensure that all nursing personnel are performing their work assignments in accordance with acceptable nursing standards. Report findings to the Director.

· Monitor absenteeism to ensure there is adequate number of nursing care personnel are on duty at all times. Report all call offs to the Scheduler/Human Resources.

· Ensure that established policies and procedures, including dress codes, are followed by department personnel.

· Develop and maintain a good working rapport with inter-department personnel, as well as other departments within the facility to ensure that nursing services and activities can be adequately maintained to meet the needs of the residents.

· Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift.

· Meet with your shift regularly to assist in identifying and correcting problem areas and/or assist in the improvement of services.

· Ensure that departmental disciplinary action is administered fairly and without regard to race, color, creed, national origin, age, sex, religion, handicap, or marital status.

· Report occupation exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility’s policies and procedures governing accidents and incidents.

· Ensure that departmental computer workstations left unattended are properly logged off or the password protected automatic screen-saver activates within established facility policy guidelines.

Nursing Care Functions

· Inform nursing service personnel of new admissions, their expected time of arrival, room assignment, etc.

· Ensure that rooms are ready for new admission. (i.e. equipment, concentrator, bipap, cpap, CPM machine,etc.)

· Greet new admissions introduce yourself and roommate if necessary

· Complete full admission in Point Click Care (i.e. complete all assessments necessary, diet orders, medication lists,etc.)

· Must get PULSE and Complete Admission packet signed upon admission.

· If resident is unable to sign pulse and admission packet must call family to get verbal consent.

· Make rounds with physician when necessary

· Encourage attending physicians to record and sign progress notes, incident reports, physician’s orders, etc., on a timely basis and accordance with current regulations.

· Consult with the resident, his/her family, and the resident’s physician in planning the resident’s care, treatment, rehabilitation, etc., as necessary.

· Notify the resident’s next-of-kin and physician when there is an accident/incident and change of condition.

· Must Notify On-Call for Abuse, Neglect or Misappropriation within 2 hours to be compliant with Department of Health Policy and Procedures.

· Review nurses’ notes to ensure that they are informative and descriptive of the nursing care being provided, that they reflect the resident’s response to the care, and that such is provided in accordance with the resident’s wishes.

· Must provide direct nursing care as necessary.

· Must start IV’S; obtain sputum, urine and other specimens for lab test as ordered.

· Must be able to take and record temperature, pulse, respirations, blood pressures, and accuchecks.

· Admit, transfer, discharge residents completely in Point Click Care.

· Must keep the Director of Nursing updated with report on residents in the building.

· Assist LPNs with caring for residents who are seriously ill with checking on their needs more frequently.

· Ensure that all RNS and LPNS on your shift comply with written procedures for the administration, storage, and control of medications and supplies.

· Monitor medication passes and treatment schedules to ensure that medications are being administered as ordered and that treatments are provided as scheduled.

· Review the MARS/TARS to ensure completion of medication pass and treatments completed and the documentation is accurate in the transcription of physician’s orders, and adherence to the stop order policies.

· Ensure that residents who are unable to call for help are checked frequently.

· Meet with residents, and/or family members, as necessary. Report problems to the Director.

· Assist in arranging transportation for discharged residents as necessary.

· Inform family members of the death of the resident, call funeral home when requested by family.

Staff Development

· Must attend and participate in continuing education programs designed to keep you abreast of changes in your profession, as well as maintain your license on a current status.

· Attend and participate in outside training programs.

· Attend and participate in monthly/annual facility in-service training programs as scheduled.

Safety and Sanitation

· Ensure that unit is free of debris and trip hazards and remains clean at all times.

· Monitor your assigned personnel to ensure that they are following established safety regulations in the use of equipment and supplies.

· Ensure that established departmental policies and procedures, including dress codes, are followed by your assigned nursing personnel.

· Ensure the personnel follow established procedures for the use and disposal of personal protective equipment.

· Ensure that all nursing service personnel comply with established departmental policies and procedures.

· Ensure that nursing personnel follow established infection control procedures when isolation precautions become necessary.

· Ensure that the adequate supply of personal protective equipment is on hand and are readily available to personnel who performs procedures that involve exposure to blood or body fluids.

· Ensure that your unit’s resident care rooms, treatment areas, etc., are maintained in a clean, safe, and sanitary manner.

· Ensure that your assigned personnel follow established handwashing and hand hygiene technique in the administering of nursing care procedures.

· Ensure that all personnel wear and/or use safety equipment and supplies (i.e. back brace, mechanical lifts, proper lifting technique, etc.) when lifting or moving residents.

· Have the knowledge of what is in the MSDS binders, report missing/illegible labels and MSDSs to the safety officer or other designated person.

Care Plan and Assessment

· Review care plans daily to ensure that appropriate care is being rendered.

· Inform the RNAC of any changes that needed to be made on the care plan.

· Ensure that your nurses’ notes reflect that the care plan is being followed when administering nursing care or treatment.

· Review resident care plans for appropriate resident goals, problems, approaches, and revisions based on nursing needs.

· Ensure that all personnel are aware of the residents care plan. Ensure that the nursing personnel refer to the resident’s care plan prior to administering daily care to the resident.

· Must complete all assessments that are designated to you by the director of nursing

· Encourage the resident to participate in the development and review of his/her care plan.

SPECIFIC REQUIREMENTS:

· Must be able to read, write, speak, and understand the English language

· Must be a supportive team member, contribute to and be an example of team work and the team concept

· Must possess the ability to make independent decisions when circumstances warrant such action.

· Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public

· Must be able to follow written and oral instructions

· Must not pose a threat to the health or safety of their individuals in the workplace

· Must be able to move intermittently throughout the workday

· Must be able to function independently and have flexibility, personal integrity, and the ability to work effectively with other

· Must be willing to perform tasks that may involve exposure to the blood and/or body fluids

· Must be willing to assist in the evacuation of residents during emergency situations

· Perform any and all other job duties assigned to you by supervisor
