QUALIFICATIONS:

· Must possess a GED or High school Diploma

· Must be licensed Certified Nursing Assistant in accordance with the laws of this state.

· Must have effective oral and written communication skills

· Must meet all local health regulations and criminal background checks, post-offer drug test, and post-offer physical exam, if required

· Must maintain all required licenses, certifications, and continuing education requirements

· Must be able to obtain and record vital signs manually and electronically

ESSENTIAL RESPONSIBILITIES AND DUTIES:

· Record all charting into Point Click Care accurately and appropriate manner

· Report all changes/incidents in the resident’s condition to the Nurse Supervisor/Licensed Practical Nurse immediately.

· Maintain confidentiality of usernames and passwords for accessing resident/facility information and promptly report suspected or known violations of such disclosure to the Administrator.

· Perform all assigned tasks in accordance with our established policies and procedures, and as instructed by your supervisors.

· Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift

· Required to attend all Staff Meetings per the Employee Handbook

· Report any potential or actual safety hazards to Maintenance and Supervisor

· Report all complaints and grievances made by resident to Supervisor and Administration.

· Must work as a team member and well with all coworkers

Resident Rights

· Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.

· Ensure that you treat all residents fairly, and with kindness, dignity, and respect

· Ensure that all nursing is provided in privacy

· Knock before entering the resident’s room

· Report all grievances and complaints made by the resident to the Nurse Supervisor/Licensed Practical Nurse

· Report all allegations of resident abuse and/or misappropriation of residents property immediately to the Nurse Supervisor

· Honor the resident’s refusal of treatments request. Report such requests to your supervisor

· Communicate respectfully and appropriately, respect culture differences among residents and staff

Admission/Transfer/Discharge of a Resident

· Ensure that a residents room is ready for receiving the resident (i.e. bed made, name tags up, admission kit available, etc.)

· Greet the new admission and introduce yourself and/or roommate

· Fully complete Admission packet (i.e. weigh resident, check for dentures, eye glasses, hearing aide, personal items)

· Label Resident clothing/Put away all residents belongings

· When transferring resident ensures that device used to transport such as wheelchair, walker, etc. is in good working order for resident’s safety.

· Responsible for packing resident’s belongings for transfer and/or discharge from facility

Personal Nursing Care of Resident

· Participate and receiving report/giving report to oncoming shift

· Follow established policies concerning exposure of blood/body fluids, infection prevention and infection control.

· Assist residents with daily dental and mouth care (i.e. brushing teeth/dentures, oral hygiene, special mouth care, etc.)

· Assist residents with bath functions (i.e. bed bath, tub or shower bath, etc.) as directed

· Assist with dressing/undressing as necessary

· Assist with hair care functions (i.e. combing, brushing, shampooing, etc.)

· Assist residents with nail care on shower days and PRN (i.e. clipping, trimming, and cleaning finger/toenails). (NOTE: This does not include diabetic residents.)

· Shave residents daily or shower days. (As requested)

· Assist with maintaining resident’s skin integrity by performing incontinence care as necessary, assist resident with bowel and bladder functions (i.e. take to bathroom, offer bedpan/urinal, bedside commode, etc.).

· Maintain intake and output records as instructed.

· Must make sure bed is properly made avoid having wrinkles in resident’s bed.

· Position residents in correct comfortable positions whether in bed, wheelchair, recliner and etc. Reposition resident per plan of care.

· Must chart bowel movements and urine outputs in Point Click Care (i.e. Foley outputs, regular restroom use, character of stools as instructed.)

· Perform restorative nursing care for residents (i.e. walking residents, range of motion to upper and lower extremities.)

· Measure and Record temperatures, pulse, and respirations (TPRS), as instructed.

· Must weigh and record the resident’s weight accurate with using correct scale and wheelchair weight.

· Must ensure residents call bells are within reach and are answered in appropriate amount of time

· Check on residents frequently to ensure needs are met according to plan of care

· Assist with care of dying resident by providing compassionate and supportive care to resident and family as appropriate

· Must be able to provide post mortem care as needed

Special Nursing Care Functions

· Observe and report the presence of pressure areas and skin breakdowns to prevent decubitus ulcers (bedsores).

· Report all skin tears, bruises and blisters to immediate charge nurse

· Provide daily indwelling catheter care

· Provide daily perineal care

· Assist with application of slings, braces, and assistive devices.

· Turn all medications found in the resident’s room/possession over to the Nurse Supervisor/Licensed Practical Nurse.

· Basic communication skills how to speak and communicate respectfully.

· Basic knowledge of normal and abnormal aging sensory changes and relationship to resident safety

· Basic knowledge of providing care to residents with dementia aggressive assaultive behavior to ensure resident and staff safety including wandering behavior.

Food Service Functions

· Prepare resident for meals (i.e. take to bathroom, wash hands, comb hair, raise bed, position tables, position resident in wheelchair/bed, place clothing protectors, take to/from dining room, etc.).

· Serve food trays. Assist feeding as indicated (i.e. ensure correct food tray, consistency for correct resident, cutting foods, feeding, assist in dining room supervision, etc.)

· Assist residents with identifying food arrangements (i.e. informing resident with sight problem of foods that are on his/her tray, where it is located, if it is hot/cold, etc.)

· Record the resident’s food/fluid intake, report changes in the resident’s eating habits to immediate supervisor.

· Keep resident’s water pitchers clean and filled with fresh water (on each shift), and within easy reach of the resident.

· Serve between meal and bedtime snacks and record consumption

· Perform after meal care (i.e. remove trays, clean resident’s hands, face, clothing, take to bathroom, brush teeth, clean dentures, remove excess food from clothing/bedding etc.)

· Understand importance of providing all hydration and nutrition to older adults

Staff Development

· Must stay compliant and up-to-date on Relias education for the 12 hours required by state to maintain CNA certification

· Must attend and participate in scheduled mandatory in-services and education

Safety and Sanitation

· Maintain appropriate hand hygiene

· Utilize and understand proper personal protective equipment and when is appropriate to use

· Properly dispose of PPE equipment after use

· Immediately notify the Nurse Supervisor/Licensed Practical Nurse of any resident leaving/missing from facility

· Keep floor from having excess supplies and equipment. Store in designated areas

· Clean, disinfect, and return all residents care equipment to its designated storage area after each use. (i.e. shower chairs)

· Report all safety and hazardous conditions and equipment to the Nurse Supervisor/Licensed Practical Nurse immediately.

· Wear and/or use safety equipment and supplies (i.e. back brace, mechanical lifts, etc.) when lifting or moving residents.

· Report missing/illegible labels on cleaners/disinfectants and MSDS to your supervisor

· Must have knowledge on how to read and identify MSDS binder

Equipment and Supply Functions

· Must be able to use all equipment and have the knowledge of how the equipment works (i.e. Hoyer’s, sit to stands, bath chairs, wheelchairs, and mechanical beds.)

· Use supplies mindfully DO NOT be wasteful (i.e. shampoo, body wash, shaving cream, and razors etc.)

· Report defective equipment to the Nurse Supervisor and Maintenance

· Inform Nurse Supervisor/Licensed Practical Nurse of your equipment and supply needs to perform your job duties to the fullest

SPECIFIC REQUIREMENTS:

· Must be able to read, write, speak, and understand the English language

· Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.

· Must be a supportive team member, contribute to and be an example of team work and the team concept

· Must possess the ability to make independent decisions when circumstances warrant such actions.

· Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public

· Must be able to follow written and oral instructions

· Must not pose a threat to the health or safety of their individuals in the workplace

· Must be able to move intermittently throughout the workday

· Must be able to function independently and have flexibility, personal integrity, and the ability to work effectively with other

· Must be willing to perform tasks that may involve exposure to the blood and/or body fluids

· Must be willing to assist in the evacuation of residents during emergency situations

· Perform any and all other job duties assigned to you by supervisor

Job Type: Full-time

Pay: $20.70 - $24.93 per hour
