Job Title: Hospice RN Case Manager
Duties & Responsibilities: Hospice RN Case Manager sought for 12 week assignment, with the potential to extend. Schedule would be 5x8 day shifts Monday-Friday. On call is required, weekend and after-hours weekday on call is split between other RNs. The area of coverage is Columbia, SC area. FAcility will keep visits centralized as much as possible.
Minimum Requirements:
- SC or Compact RN license
- 1 year hospice in home settings experience within the past 3 years
- BLS
- HCHB experience
- Competitive Pay and Benefits
- 401k eligibility after 6 months of employment
