This role is vital to the company as it is the face of our vision, mission, and culture. Alopex's Care Coordinators interact remotely with our client's patients on a daily basis ensuring that extra effort and care management they need on their health journey. We are seeking a dynamic CMA, CNA, or LPN who is fluent in Spanish to work with our non-English speaking patients in multiple parts of the country. The Spanish speaking Care Coordinator would should be bi-lingual in English and Spanish. Their daily duties include: Conducting a detailed review of EMR records, developing and maintaining individualized Care Plans for management of patients, implementing and coordinating all care management activities relating to patients across the continuum of care, communicating with the patient’s clinical care team. Care management must be performed in a timely manner, monitoring patient progress toward desired outcomes through assessment and evaluation, and maintaining accurate and detailed documentation related to medication, problems, goals, interventions, preventative care, etc.
The Care Coordinator will be conducting monthly care calls to the patients enrolled in the programs, addressing all needs/concerns including any follow up needed for patients, educating patients on chronic conditions and reinforce the provider’s directions related to chronic conditions, evaluating and addressing social determinants to health and begin to link community resources; then, enter them in the system, identifying medical conditions and addressing needed changes in a clinically-appropriate manner, and adhering to all policies & procedure as outlined by Medicare and Alopex.
This role must commit to working a M-F 8-5pm EST like schedule and have a stable, quiet, and professional home office environment with stable internet service.
