Alopex is looking a dynamic Care Plan nurse to work entirely remote creating high quality, initial care plans for patients who have consented to various services provided by Alopex (i.e. Chronic Care Management, Principal Care Management, and Behavioral Health Integration). This pivotal role can be per diem or part time and can work up to full time eventually if desired. We desire a candidate with care planning experience, high attention to detail, and a good attitude who will also participate in the ongoing care plan review processes. The Care Plan Nurse can be a licensed RN or LPN (valid active license-Compact license preferred) and will also need to participate in the documentation review process with the QA team.
This nurse must have proficiency with medical record systems and thorough review of medical records with the ability to create high-level, detailed individualized patient care plans for Alopex's Provider clients' patients. Care plans must follow the company's outlined guidelines and requires the candidate to work within a team of care coordinators who will execute the care plans with the patient. The job requires you monitor the patient progress towards the goals outlined in the plan, maintaining detailed and accurate records and providing assessments and evaluations remotely. This nurse must understand CCM/PCM/BHI/TCM/RPM/RTM program enrollment requirements and be able to quickly evaluate a patient's eligibility for these programs.
The role requires at least 2 years of clinical, bedside experience, experience with developing care plans and providing clinical assessments. Candidate must have high proficiency with software programs, be adaptable to new technology, and have extreme attention to detail. You must be able to work independently and have a high desire to achieve. You must have a professional, quiet home office environment that allows for privacy with excellent, stable WIFI access.
