Alpha Home Health and Hospice is seeking an experienced and caring full-time LPN (LVN, Licensed Practical Nurse) Home Health Coordinator for our Home Health division supporting Snohomish County in our Everett office.

Essential Duties

• Clinical review of Home Health referrals for eligibility, level of care, and urgency
• Responds to inbound calls regarding referrals
• Coordinates referral workflow with marketing associates and clinical managers
• Builds and maintains positive relationships with referral sources and community partners
• Conferences with Attending Physicians regarding patient eligibility for services
• Participates in quality assessment improvement teams and activities
• Reviews HH F2F for eligibility/ to ensure criteria met
• Provides recommendations to DCS on in-service and continuing education
• Attends weekly Clinical Accountability meeting
• Processes workflow as needed, including entering/transcribing/approving lab and wound care orders
• Works with referral source platforms and faxes to review incoming referrals
• Assists with determining clinician assignments based on admission criteria
• Maintains knowledge of and assists referral partners with community resources should services not be available from Alpha Home Health
• **Must be available for PRN on-call for after-hours consultations and possible in-home visits to assist with staff vacation, illnesses, and other emergencies.
• When completing in-home visits the following duty expectations would apply:
• Provide one-on-one skilled nursing care to patients in their homes (e.g., wound care, catheter care, medication administration, symptom management, etc.)
• Provide skilled observation, assessment, and evaluation
• Provide patient and family/caregiver support and education
• Practice cohesive and collaborative care with the healthcare providers

Requirements and Qualifications

• Washington LPN (LPN) licensure
• Prior Home Health experience is required
• Current BLS certification from AHA
• Be a licensed driver with an insured automobile in good working order
• Prefer knowledge of HCHB/Point Care (not Point Click Care)
