Altura Centers for Health is Hiring a COMMUNITY HEALTH WORKER- Cal-AIMS Near Tulare, CA

Statement of Purpose

The purpose of the grant is to implement public health inrural communities to provide outreach to the most vulnerable and at-risk populations, low-socioeconomic populations, farmworkers, homelessness, rural and limited English speaking communities. Educate patients, community members and link them to services they need. Identify and manage comprehensive needs through whole person care approaches and social drivers of health.

Essential Functions, Knowledge, Skills and Abilities: CAL AIM (60% FTE)

  • Promote health equity by providing culturally competent services
  • Will support crucial transitions, including the path from homelessness to housing, from incarceration to community re-entry, and from institutional to home-based care.
  • Will participate or host community events, provide classes, social media advertisement to promote awareness of resources to engage them in community supports.
  • Will collaborate with other partnered organizations through community outreach service events providing educational information about Community Supports
  • Actively participates in community collaboratives such as Tulare County CalAIM Collaborative and PATH CITED Collaborative Planning and Implementation Committee
  • Promote increased community awareness of the services provided at Altura Centers for Health as a Community Support for health needs.
  • Participate in all Altura Outreach Events and health fairs, and other community projects with the goal of increasing patient population, increasing awareness about patient programs and clinic services as a Community Support for health needs.
  • Supports the Promotora program by following up with individuals who have identified needs for Community Supports.
  • Refer uninsured individuals to Altura Centers for Health Certified Enrollment Counselors for enrollment in health plans
  • Advises and makes recommendations to physicians, patients and families regarding the availability of resources and coverage, locates the resources, and arranges for the provision of cost effective services for patients.
  • Maintains a record of patients served that includes all support services and efforts made to resolve issues of care.
  • Provides presentations on ALTURA to church groups, local community groups, and any service organizations to educate the community on ALTURA services.
  • Attends and collaborates with local collaborative, consortia, special interest groups, etc. to help maintain resources and useful information for our patients.
  • Assists patient in filling out required documents for Community Service programs as needed.
  • Attend home visits with Altura Center for Health’s Enhanced Care Management Coordinator.
  • Other functions as needed to support outreach to populations who are traditionally underserved to engage them in Community Supports.

Essential Functions, Knowledge, Skills and Abilities: CHW responsibilities (40% of FTE)

  • Educate clients about chronic disease prevention and management, especially diseases known to impact COVID-19 outcomes.
  • will deliver training to increase the skills of CHWs to educate patients and community members
  • Enhance education for patients, families and communities to support the use of contraceptives to reduce unplanned pregnancy.
  • Provide health education relating to teen pregnancy, STD and adolescent health.
  • Teach the teen pregnancy prevention curriculum at various schools during the year when requested by local school districts.
  • Promote increased community awareness of opioid abuse prevention and treatment resources.
  • Enhance education for patients, families and communities to support patient engagement and self-management that includes medical conditions that often co-occur with mental health and substance use disorders (eg: diabetes, hypertension, obesity).
  • Collaborate with existing community resources to educate the community about opioid use disorders.
  • Participates in community medical screenings; take patient vitals (temperature, blood pressure, height, and weight) and perform glucose and hemoglobin screenings. Assist with dental screenings as needed.
  • Supports the Promotora program
  • Will assist patient in outreach and education on the Affordable Care Act Health Exchange “Covered California” health insurance plans, Medicaid expansion or CHIP and Medi-Cal services.
  • Will follow all the standards and requirements from Covered California and State Medicaid.
  • Serves as a liaison for the health team, patient and family.
  • Serves as a patient advocate providing support and a voice for patients who cannot effectively do this on their own.
  • Will provide case management (non-clinical) to patients to ensure they are following through with their prescribed treatment plan.
  • Ensures referrals to the county eligibility worker for patients that are eligible for State sponsored programs.
  • Works closely with the health educator to provide education regarding disease processes, disease prevention and treatment.
  • Accurately completes patient information for patient programs
  • Ensures a timely response of information for the prescription assistance program.

General Essential Functions:

  • Ensures that communication with staff, patients and external contacts occur with courtesy, tact and a positive attitude.
  • Participates in relevant staff training such as orientation and staff development meetings.

  • Professionally handles patient complaints.
  • Meets all attendance and punctuality requirements to ensure proper coverage and quality service.
  • Professional and appropriate dress as required by the position and compliant to the ALTURA dress code.
  • Demonstrates an ability to resolve interpersonal and professional conflicts appropriately.
  • Participates in staff meetings, trainings and committees.
  • Keeps all matters related to the organization confidential in compliance with confidentiality policy.
  • Takes pride in job performance as evident in compliance with job responsibilities.

  • Develops trust and teamwork with staff and patients.

Required Education and Experience

  • Minimum Education: High school graduate of GED equivalent required.
  • One year experience in Ambulatory Care setting.
  • Bilingual in English and Spanish is required.
  • Valid California Driver’s License and insurable through Altura policy.

Work Environment

Office environment with controlled temperature. Outdoor environment with outdoor weather conditions.

Position Type/Expected Hours of Work

This is a full-time position. Days and hours of work are Monday through Friday, 8:00 a.m. to 5:00 p.m. Occasional evening and weekend work may be required as job duties demand.

Travel

Travel is expected for this position. Typical example of travel would be driving to Altura’s various clinic locations, event or trainings.
