Responsibilities

•Coordinate post discharge needs for patients leaving the skilled nursing facility, including home health, hospice, palliative care, placement referrals, chronic care management, remote patient monitoring, and other community agency referrals as appropriate.•Advise operations departments on process and business opportunities based upon field experience.

Qualifications

•High School diploma or GED equivalent.•A minimum of six (6) months experience in office administration required.•Related experience in a healthcare setting preferred.•Must be willing and able to learn various electronic medical and health record systems.•General knowledge and understanding of Medicare Benefits preferred.•General knowledge and understanding of Hospice, Home Health and Skilled Nursing preferred.
