Amergis Healthcare Staffing is Hiring a Social Worker - LMSW/LBSW - Baltimore, Maryland Near Baltimore, MD

Amergis Staffing is currently seeking highly skilled Medical Social Wworkers for an assignment in Baltimore, MD.
Eight LMSW or LBSW's needed!
Contract DetailsCompensation: LMSW's will be $45-$48/hr - LBSW's will be $40-$43/hr. Start Date: ASAPContract Length: 26wksLocal Candidates PreferredKEY NOTES:
  • Social Workers (LBSW and LMSW licensures) will be performing Community assessments as part of the AERS Program in Baltimore City
    • Adult Evaluation & Review Services - program conducts comprehensive evaluations that identify the psychosocial and medical needs of older adults and adults with disabilities
  • Location of Assessments – Baltimore City area, assessments are scheduled at safe community centers (nursing homes, assisted living, etc.)
  • Equipment – Candidates will need to have their own laptop, older chrome books and MacBook’s accepted as long as connectivity works to the LTSS Maryland portal.
    • all PHI is entered into the LTSS portal. There will not be any documents or information entered anywhere else on staffs personal laptops.
  • Schedule – Monday – Friday business hours, flexibility on schedule as you get acclimated.
    • Will work with the program manager for weekly scheduling
  • Duration: 6 months to start with possibility to extend
  • Mileage reimbursement is offered: You would complete a mileage log and get supervisor approval and sign off weekly (standard IRS rate - $0.67 per mile)
Essential Duties
  • Review State Tracking System (LTSS) data/interRAI assessments and any attachments upon assignment of cases.
  • Contact assigned clients/representatives timely.
  • Develop a plan of care for services to be rendered
  • Complete all documentation via LTSS timely and in keeping with Program policies/processes.
  • Performs other related duties as required.
The Medical Social Worker collaborates with the multidisciplinary team to provide input in the development of the plan of care for those patients requiring social work intervention. The Medical Social Worker provides medically necessary intervention to patients who have social or emotional problems that are, or are expected to be, impediment(s) to the effective treatment of the medical condition and/or rate of recovery. Additionally, the Medical Social Worker provides these services in accordance with physician’s orders.
Responsibilities
  • Provides medically necessary social work intervention in accordance with physician’s orders
  • Assesses patient’s psychosocial needs; plans for, evaluates, and modifies interventions to meet the established patient goals
  • Collaborates with other disciplines to plan and evaluate team goals for each patient
  • Facilitates patient referrals to community agencies when appropriate
  • Provides financial counseling when necessary to achieve the treatment goals established by the multidisciplinary team
  • Assists patient/family/caregivers in planning for present and future psychosocial needs related to the patient’s current status and prognosis, including terminal illness
  • Adheres to company policies and procedures while providing services and preparing/submitting required paperwork
  • Observes and reports patient progress and/or changes in patient status to physician and appropriate multidisciplinary team members
  • Functions as a resource person for other members of the health care team, providing inservice education and consultation in understanding the social and emotional factors related to health problems.
  • Maintains knowledge of the field including Medicare, Medicaid, and other third-party reimbursement sources.
  • Participates in planning and implementing quality improvement activities as necessary
  • Provides positive experiences in assisting patients, families, physicians, other departmental employees, referral resources, service agencies, and others.
  • Adheres to facility and Amergis policies at all times
Minimum Requirements
  • Baccalaureate degree in Social Work (BSW) or Master’s degree from a school of social work accredited by the Council on Social Work Education per state/client requirements required
  • Minimum of one (1) year of social work experience in a health care environment in the last two years preferred
  • Complies with all relevant professional standards of practice
  • Participation and completion of Amergis' Competency program when applicable
  • Current CPR if applicable
  • TB questionnaire, PPD or chest x-ray if applicable
  • Current Health certificate (per contract or state regulation)
  • Must meet all federal, state and local requirements
  • Successful completion of new hire training as applicable to job site
  • Understand patient confidentiality and HIPAA requirements
  • Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required
  • Computer proficiency required
  • Must be at least 18 years of age
