RN Case Manager-Home health care

American Care Partners (ACP), a State Licensed, Medicare & Medicaid certified, and Joint commission (JACHO) Accredited Home Health Care Agency serving Falls Church Fairfax, and all five counties in Northern Virginia area for personal care services, Private Duty In-Home Nursing Services, and Skilled Nursing.

We are looking to add an RN-Pediatric Home Health Care Case Manager to become part of our team to conduct supervisory visits for our current clients and conduct skilled nursing visits in the homes on recently discharged pediatric patients. The goal of these visits are to educate patient caregivers, perform physician ordered tasks, conduct comprehensive nursing assessments, communicate with the physician and to overall, prevent hospital readmission.

Duties:

  • Enforcing compliance with the standard of care plan.
  • Updates plans of care as necessary
  • Assist in interviewing and orienting new hires
  • Comprehensive Nursing Assessments
  • Conducting Supervisory Visits
  • Create educational materials

Qualifications:

  • Current, state of Virginia license to practice as a Registered Nurse (RN)
  • Minimum 1 year of Pediatric nursing experience required
  • Excellent customer service attitude and passion for working with Children is a must!
  • Current CPR certification
  • Copy of updated Negative TB results/ Chest x-ray and Immunization.
  • Reliable transportation

  • Comfortable working with sick and fragile children
  • Excellent customer service attitude and passion for working with Children is a must!
  • Team player and ability to resourcefully solve problems
  • Ability to learn and comply with the various regulations of the home care Industry
