Community Health WorkerReports To FHW Program CoordinatorJob Overview
The CHW embraces a holistic approach to addressing the physical, social, and behavioral determinants of health, Collectively, these are integral to achieve personalized care and improved outcomes.
Responsibilities and Duties

  • Mentors clients/participants
  • Maintains confidentiality of information
  • Provides client outreach, facilitates resources and conducts client surveys
  • Facilitates/Co-facilitates classes
  • Conducts client intake
  • Attends compliance training
  • Maintains regular communication with wellness team
  • Performs other duties as assigned

Qualifications

  • High School Diploma or GED
  • Texas DL and proof of insurance
  • Community Health Worker Certificate (Preferred)
  • Ability to lift 5-10 pounds frequently, 10-50 pounds occasionally

Job Types: Full-time, Part-time

Pay: From $15.00 per hour

Expected hours: 10 – 40 per week
